General Terms and Conditions

Navani Menopause clinic is a private health clinic and referral from your GP/consultant is not required. However in some cases Dr Navani may request additional information from your GP about your medical conditions and tests . Please let her know if you don’t want us to approach your GP.

Consultation fees must be paid in full prior to the consultation via BACS transfer or credit card.  

Additional charges will have to be paid to the laboratory for investigations, and for prescriptions or extra letters as required.

Emergency care, out of hours service is not provided by this clinic.

 Dr Navani reserves the right to decline access to the services

This service has a zero tolerance for any violent , threatening or abusive behaviour .

Cancellation and pricing policy

Please  try and give as much notice as possible if you wish to cancel your appointment, to allow this appointment slot to be offered to another patient .

You can cancel at any time via the Heydoc appointment system.

Full refund will be provided for appointments cancelled with more than 48 hours notice.

Cancellations requested with less than 48 hours notice will be given a 50% refund.

The fee structure is subject to change without notice but you will be charged the applicable fees at the time of your booking.

 

Complaints policy

The clinic is committed to providing the highest standard of care to every patient. Constructive feedback on your visit is always welcome as Dr Navani strives to improve the patient experience.

Complaints about the service can be sent by e mail to [email protected]

Your complaint will be acknowledged within 3 working days. A full investigation will be carried out following which you will be contacted to discuss and resolve the issue/s. You will receive a full written response within 20 working days of the complaint being received. If a full response cannot be given within 20 working days, you will be informed explaining  the reason for the delay.

In certain cases a refund may be given.

Patients can lodge a complaint with the Information Commissioners Office

(ICO) , if they feel their rights such as storage, use and processing your data  have been breached at https://ico.org.uk/make-a-complaint/

I would, however, appreciate the chance to deal with your concerns before you approach the ICO so please contact me in the first instance.

  • 2 factor authentication for users login with SSL encryption (the same level of security as used for online banking)

When do we collect personal data about you?

We may collect personal data about you if you:

  • visit this website
    • when you  interact with the clinic through the website, by phone,

e mail, social media or in person

    • carry out a transaction on our website, or order prescription, book blood tests or other investigations
  • participate in interactive features on our websites.

 

What data do we collect?

We collect information that you give us when you enquire or become a customer or patient of us includes the following

  • name, address, contact details (including email address and phone number)
  • details of referrals, and other contact and correspondence details
  • details of services and/or treatment you have received from us
  • recordings of calls we receive or make
  • notes and reports about your health and any treatment and care you have received and/or need, including about clinic and hospital visits and medicines administered
  • patient feedback and treatment outcome information you provide
  • information about complaints and incidents
  • information you give us when you make a payment to us, such as financial or credit card information
  • Where you have named someone as your next of kin/emergency contact and provided us with personal data about that individual, it is your responsibility to ensure that that individual is aware of and accepts the terms of this Privacy Policy.

The data that we request from you may include sensitive personal data. This includes information that relates to the mental or physical health or racial or ethnic origin (which may include children’s data).

By providing us with sensitive personal data, you give us your explicit consent to process this sensitive personal data for the purposes set out in this Privacy Policy.

This information may be used to

  • ensure you receive the best possible care and to review the care provided
  • investigate adverse incidents and complaints
  • perform clinical audit to monitor and improve the quality of care and services provided

 

Information sharing will be done with your consent. We maintain our duty of confidentiality to you at all times

There are certain  situations where information may need to be passed on to others and these include

  • when others involved in the patient care have a genuine need for the information
  • in exceptional circumstances when the health and safety of others is at risk or where the law requires information to be passed on
  • where information is required for national registries
  • when required by law to report certain information to the appropriate authorities

Local information sharing : You will be asked whether you consent to documentation about your contacts in the service being shared with your NHS GP or any secondary care health professionals involved in your care.

In all cases, your information is only accessed and used by authorised staff who are involved in providing or supporting your direct care.

You can object to your personal information being shared with other healthcare providers but should be aware that this may, in some instances, affect your care as important information about your health might not be available to healthcare staff in other organisations. If this limits the treatment that you can receive then the clinic staff will explain this to you at the time you object.

Safeguarding

To ensure that adult and children’s safeguarding matters are managed appropriately, access to identifiable information will be shared in some limited circumstances where it’s legally required for the safety of the individuals concerned.

Do we share your data with any third parties or subcontractors ?

We do not share your data with any third parties without your consent, except we  may pass your information to agents, subcontractors with the purpose of them providing services to you.

What is the time period for storage of your data

The time period that we will keep information for will vary depending on what the information is used forUnless there is a specific legal requirement to the contrary, we will keep information in a form which permits identification of Data Subjects only for as long as it is necessary for the purposes for which we process it.  Once the requirement to hold the data is complete, appropriate measures will be taken to delete the data in line with the terms of the GDPR. Data maybe processed for audit purposes , but in such cases it will be anonymised.

 

Your rights to your personal data

Under the Data Protection Act 2018 everybody has the right to see, or have a copy, of data we hold that can identify you, with some exceptions. You do not need to give a reason to see your data. Under special circumstances, some information may be withheld.

Your request must be made in writing (which can be by email).

You will receive a response within 30 days.

You will need to give adequate information (for example full name, address, date of birth, reference number and details of your request) so that your identity can be verified and your records located.

There will be a fee for the administration of the request in certain instances (e.g. where a duplicate copy is requested).
